DLSU - Senior High School Application Form

1 Application Period Application for Academic Year 2024-2025 is from July 15, 2023 to October 15, 2023.

2 Registration You will have to create an account using a valid email address. Please do not forget your username and password as they will be your access throughout the application process.

3 Application Requirements Please clearly scan and upload the following:

Consent Form (Download here)
2x2 size most recent photo (white background)
PSA/NSO copy of Birth Certificate
Junior High School Scholastic Record (Download here)
Valid ID (School ID or any government ID)

Additional Requirements for:

Filipinos with dual citizenship/Born in the Philippines or abroad - Valid Passport, Documentation for Acquisition/Reacquisition/Retention of Filipino Citizenship
Non-Filipinos/Born in the Philippines or abroad - Valid Passport
Students with other visa such as 47a2, 9G etc. - Photocopy of Passport bio-page and stamp of latest Philippine arrival, scanned copy of student and parent/s i-Card (front and back), SRRV, SIRV AND Working Visa card

4 Application Fee The application fee is Php600.00 but is waived for applicants coming from public schools.

5 Submission Please ensure completeness and accuracy of all information entered. You will receive a confirmation email once the application has been successfully submitted.

6 Verification Your application and all uploaded documents will undergo a verification process to determine if requirements are complete. Once verified, an email will be sent to your registered email address. To ensure receipt of such, we highly suggest that you add seniorhigh@dlsu.edu.ph in your address/contacts list.

7 Test Venues/Dates MANILA campus - Sunday November 26, 2023. LAGUNA campus - Sunday November 19, 2023. Provincial Testing Centers - To be announced soon.

8 Results The date of release of the admission results will be announced through the DLSU website at https://www.dlsu.edu.ph.
